Effective Immediately - Ownership updates are required for all currently enrolled providers.

  • All currently enrolled providers (individual and businesses) are required to update ownership data whenever there are owner, management, agent, and/or subcontractor changes.
  • The Provider Ownership Enrollment web application is designed to ask the necessary questions, which allows the ownership data to be processed securely, quickly and efficiently.
  • Providers may use the Provider Ownership Enrollment web application to update ownership here:
